Privacy Act 1988 Disclosure Acknowledgement and Consent

Information about your identity is apparent or can reasonably be ascertained (“your personal information”) will be collected by Tilmak Pty Ltd trading as the Money Centre Richmond “the Broker” when you complete and submit this application. It is necessary for the Broker to collect your personal information in o*rder to consider your application for a
Loan and, if approved, to administer your loan contract.

The Broker will use your personal information for this purpose, for the purpose of giving you information about other products or services the Broker may offer to you, and as otherwise authorized or required by law. If you do not provide your personal information we may be unable to consider your application for a loan. On and from 21 December 2001, in most cases you may gain access to your personal information on request. The Broker will disclose your personal information as indicated below or as otherwise authorized or required by law.

Notice of disclosure to Credit Reporting Agency

I acknowledge that the Privacy Act allows the Broker to give a credit reporting agency certain personal information about my application for credit for the purpose of obtaining a credit report about me or to allow the credit reporting agency to create or maintain a credit information file containing information about me. The information includes:

• Permitted identity details about me that allows me to be identified
• The fact that I have applied for credit and the amount of credit
• The fact that the Broker is a current credit provider to me
• Any loan repayments which become overdue by more than 60 days and for which collection has commenced
• Advice that loan repayments are no longer overdue in respect of any default that has been listed
• That cheques drawn by me for an amount not less than $100 have been dishonored more than once
• Information that, in the opinion of the Broker, I have committed a serious credit infringement
• That credit provided by the Broker that had previously been notified to a credit reporting agency has been discharged.

Obtaining Credit Reports and Credit Information

If the Broker considers it relevant to assessing this Credit information, I authorized the Broken to obtain:

• A consumer credit report from a credit reporting agency for the purpose of assessing my application for commercial credit
• Information about me from a credit reporting company agency or business which reports on the commercial credit worthiness of persons for the purpose of assessing my application for consumer credit.

I agree that the broker may exchange information about my credit arrangements (which can include information about my credit worthiness, credit history, credit standing and credit capacity) with any credit providers named in this application or named in a consumer credit report issued by a credit reporting agency. I understand that this information may be used to assess a credit application by me, to assess my credit worthiness, to exchange information with other credit providers as to the status of this loan where I am/wc are in default with other providers and to notify other credit providers of any default by me.

Guarantor I authorize the Broker to give to guarantor or intending guarantor of the purposed credit facility such information about my credit worthiness, credit standing, credit history and credit capacity for the purpose of allowing the guarantor or intending guarantor to decide whether to act as a guarantor or to keep the guarantor informed about the guarantee.

I would like to make my name available to the Broker’s associated organizations and companies involved in this direct marketing of products and services offered by the Broker or such associated organizations or companies (other than my credit information) so that they may contact me about products and services that may interest me. By signing this Application I consent to being contracted. I understand I can contact the Broker at any stage to withdraw my consent.

Bankers’ Opinions I understand that the Broker may obtain from banks a bankers opinion about my credit worthiness

Declare that the information about me/us set out above is true and correct;

Acknowledge that the Broker will rely on this information in considering my application for credit;

Acknowledge, agree and consent to all matters out above.
